There maybe a few more that are important:
/etc/pam.d/rsh: I needed a line
auth sufficient pam_rhosts_auth.so
Furthermore, if you require to run rsh without password as root you need
to add a "rsh" line to /etc/securetty.

Try "strace -f -p <pid>" where <pid> is the process id of xinetd on the
node to which you want to connect to. If you can make a connection to
that node at all you should see it in the strace output. It also
will show you whether it spawns an rshd process or not.
